All children in primary schools from Reception through to Year 6 inclusive will be offered the vaccine. Kernow Health are due to visit SRG on Wednesday 18 November.
On the letter is a link you need to follow. You will need to quote the full unique School Reference Code.
If you decide that you do not want to vaccinate your child against flu, it would help if you still complete the consent form, giving the reason (and we won’t chase you for it!).
Below is a letter that was emailed out to all parents on Saturday 16th May.
There is a questionnaire for parents of Reception, Year 1 and Year 6 children about returning to school in June. The link to the questionnaire doesn’t work for all, so this is a different link which will hopefully work.
There is also a section for key workers who will require their children to attend school from 1 June about half-way down the questionnaire.

KS1: You need to book one ten-minute slot with your child’s class teacher
KS2: You need to book one five-minute slot with your child’s class teacher AND one five-minute slot with their math’s teacher (which may or may not be the class teacher, please check before you book your appointments).
